School/Sports Physicals and Screening Information
Information regarding School Physicals:
All students in grade 7 and any student who is new to the school district need to have a physical. The student’s own health care provider can complete the physical provided the school district form is used. Any student who does not provide a physical will be examined by the School District Physician.
Information regarding Sports Physicals:
All students trying out for a school athletic team must have a current school physical on file and must submit a completed Sports Candidates’ Questionnaire prior to the first practice/ tryout. A current physical is one that has been performed after June 1 of the preceding school year. It may be completed by the student’s health care provider as long as it is completed on the school district form. All students must have a urine test for protein and glucose and a blood pressure measurement as part of the physical. The School District Physician is present in the school several days in the summer and at different times during the school year to complete sports physicals for students at no cost.
NYS Mandated Screenings:
All students in grade 7 and any student who is new to the school district will have distance vision, hearing and scoliosis screening and height and weight completed by the school nurse if it is not provided on their physical form. All students in grade 6 and 8 will be screened for scoliosis. Scoliosis screening may be completed by private health care providers if it is submitted in writing to the school nurse’s office.
